Care Planning with a Personal Touch is a 1 hour continuing education course that is intended to identify to nurses the importance of care plans and how to draft plans that reflect the personal wishes of the patient while still addressing medical conditions, psychosocial problems and potential risks for harm. The course focuses on the role of the nurse in driving a person-centered care plan and some of the challenges faced when developing person-centered care plans. It also explains how to improve the context of care plans. The course ends by differentiating person-centered versus patient-specific care plans and interventions.

Purpose

The purpose of this course is to identify to nurses the importance of care plans and how to draft plans that reflect the personal wishes of the patient while still addressing medical conditions, psychosocial problems and potential risks for harm.

Criteria for Successful Completion

To receive continuing education credit for this program, you must complete the entire course, including registration, viewing the video program, taking the post-test and obtaining a score of at least 70% or better, and completing the program evaluation. If you do not score a 70% or better, you may take the exam again as many times as necessary to receive a passing score. When a score of 70% or better is achieved, you will be able to move forward to the evaluation and receive your certificate online.

Keep this certificate in your records for four (4) years.
