Care Planning with a Personal Touch

Purpose

The purpose of this course is to identify to nurses the importance of care plans and how to draft plans that reflect the personal wishes of the patient while still addressing medical conditions, psychosocial problems and potential risks for harm.

Learning Objectives

1. Describe the role of nurses in driving patient care through person-centered care plans
2. Identify today's challenges for effectively using person-centered care plans
3. Cite the four remedies for improving the context of nursing care plans
4. Differentiate between person-centered versus patient specific care plan goals and interventions

Criteria for Successful Completion

To receive continuing education credit for this program, you must complete the entire course, including registration, viewing the video program, taking the post-test and obtaining a score of at least 70% or better, and completing the program evaluation. If you do not score a 70% or better, you may take the exam again as many times as necessary to receive a passing score. When a score of 70% or better is achieved, you will be able to move forward to the evaluation and receive your certificate online.
